Öz

Benzimidazol kimyasal sınıfına ait yeni sentetik opioidler grubu olan nitazenlerin son zamanlarda yeniden ortaya çıkması ve popülaritesinin artması halk sağlığı endişelerini artırmıştır. 1950’larda yüksek kötüye kullanım potansiyelleri nedeniyle geliştirilmesi durdurulan bu maddelerin, farmakokinetik ve farmakodinamikleri hakkında çok az şey bilinmektedir. Bu bilgi boşlukları, zehirlenmenin erken tespiti ve tedavisinde önemli zorluklar oluşturmaktadır. Protonitazen, bu sınıfta yer alan yeni ve güçlü bir sentetik opioiddir. Protonitazen, adli vaka çalışmalarında tipik olarak karşılaşılan sentetik opioidlerden (örn. fentanil, eroin) yapı olarak farklıdır. İn vitro farmakolojik veriler, bu yeni opioidin yakın zamanda ortaya çıkan diğer “nitazen” opioidlerine benzer bir güç gösterdiğini ve fentanilden yaklaşık üç kat daha güçlü olduğunu göstermektedir. Protonitazenin toksisitesi yeterince incelenmemiş veya raporlanmamıştır, ancak uyuşturucu kullanan kişilerde ölümle yakın zamanda ilişkilendirilmiş vakaların tanımlanması, bu sentetik opioidin yaygın zarara yol açma potansiyeli taşıdığını düşündürmektedir. Bu çalışmada, biyolojik numunelerinde yeni bir sentetik opioid tespit edilen çoklu uyuşturucu madde zehirlenme vakasının paylaşılması, yeni bir sentetik opioid olan, efikasite ve potensi  nedeniyle ölümcül potansiyeli yüksek olup ülkemizdeki uyuşturucu pazarına da sızma eğilimi olduğundan önem arzeden protonitazen maddesi hakkında bilgi verilmesi, nitazenler hakkında farkındalığın artırılması ve toksikoloji laboratuvarlarının bu maddelere karşı tetikte olması gerektiği hususuna dikkat çekilmesi amaçlanmıştır.
